Graduate Student Assistant Insurance

Graduate Student Assistant Medical & Dental Insurance Plan

The Graduate Student Medical & Dental Insurance Plan is provided to eligible graduate student assistants as a benefit. Please contact your appointing department to determine your eligibility. The Plan is designed to help you pay a portion of the medical & dental expenses you may incur. Coverage is available worldwide.

Eligibility: GSA Insurance Plan eligibility defined.
Domestic Partnerships: View qualifying criteria for domestic partnerships. 

 

2009-2010 Policy Information & Forms

Dependent Premiums: See Dependent Enrollment Form.
Dependent Enrollment Form: Newly acquired dependents (new spouse/domestic partner, child, adoption) must be enrolled and premium must be paid for them within 31 days after becoming eligible in order to become insured.

2008-2009 Policy Information & Forms

GSA GAP Enrollment Form: Graduate students on a summer assistantship can enroll in this plan for coverage through the end of summer until they can enroll in the 2009-2010 policy during the regular enrollment period. (See guidelines and requirements on the form.)

Important Updates

Your 2009-2010 insurance cards will be delivered through interdepartmental mail to your home department by late October for fall/annual enrollment. View more plan updates and changes.
