Enrollment Form: Newly acquired dependents (new spouse/domestic partner, child, adoption) must be enrolled and premium must be paid for them within 31 days after becoming eligible in order to become insured.
2008-2009 Policy Information & Forms
GAP Enrollment Form: International students and graduate students not on a summer assistantship - who begin their studies at WSU during the summer session - can enroll in this plan for coverage through the end of summer until they can enroll in this or the graduate student insurance plan during the regular enrollment period. (See guidelines and requirements on the form.)
